SAFARY REWARDS PROGRAM

 

Registered patients and clients who have obtained and paid for a service, procedure, product or surgery at Piscataqua Plastic Surgery & Skin Care with a combined value of $200 or more are eligible to play the Safary Rewards Program.

Each registered player receives one or more Safary Cards with their enrollment ID printed on it. The player then selects a person whom he/she wishes to refer to the practice. The player simply gives the referred party their Safary Card and asks that they turn the card in to the receptionist at the time they pay for their service and/or product.  At this time the receptionist will do two things:

• Referring to the Safary Rewards Program schedule in effect at the time the card is handed in (See below), the receptionist will give an immediate discount to the person you referred and

• Post a prepayment on the player's account (yes that's you!) for the same amount that was given as a discount to the referred party which can then be used by the player (yes, that's you again!) as a credit towards the player's next treatment,  service and /or product. Credits may also be saved and accumulated for future services.

The discounts in place at this time are:                                   

       $10 off any value of $75 or greater

       $25 off any value of $150 or greater

       $75 off any value of $450 or greater

       $100 off any value of $600 or greater

       $125 off any value of $750 or greater

       $200 off any value of $1,200 or more

 

Rules of the game:

1. These rewards may only be calculated upon, and applied against, patient/client payments and not upon payments made by an insurance company.

2.  Piscataqua Plastic Surgery reserve the right to change the schedule of discounts and prepayments at any time. The schedule in effect will always be posted on the website www.drgaudet.com

3. Safary Reward prepayments may be combined by the players to use for future purchases.

4. These rewards may not be combined with any other discount.
